LVN / LPN Travel/Contract Job Opportunity

Location: Rochester, New York
Contract Length: 13 weeks

Job Description

• Work in a 14-bed overflow med/surg telemetry unit supporting Emergency Department boarder patients.
• Unit is a flexible space inside the GI Endoscopy area; patient census may vary based on demand.
• Float required to care for med/surg patients based on census and turn.
• Weekend coverage every other weekend (EOW).
• Call requirements depend on staffing needs.
• Communication primarily via SBAR for ED patients; verbal report for other transfers/admissions.
• Previous charge experience preferred.
• Unit accepts first-time travelers.
• Scrubs color: Navy blue/white.

Required Certifications & Skills

• Certifications:

  • BLS (Basic Life Support) - AHA Provider/Instructor/RQI accepted
  • EPIC experience required
• Skills:

  • Emergency Department experience
  • Med/Surg Telemetry care
  • Blood Glucose Monitoring/Testing
  • Feeding tube care and management
  • Foley catheter insertion (male and female)
  • Ear and Eye Drops administration
  • IM & SQ injections
  • NG Tube/G Tube care
  • Oral and Subcutaneous medication administration
  • Ambulation assistance
  • Oxygen administration (face masks, nasal cannula)
  • Bathing (sitz, tub, bed, shower)
  • Mouth care
  • Patient admission
  • Communication of patient status changes to RN/MD
  • Isolation precautions
  • Vital sign monitoring

Experience Requirements

• Minimum 1 year of experience in the LVN/LPN specialty (highest requirement found).
• Prior travel experience not required.

Scheduling & Other Details

• Locals accepted only if within 75 miles.
• Pending license not accepted.
• Minimal Return to Origin (RTO) restrictions.
• Open to block scheduling but not guaranteed.

Health & Compliance Requirements

Candidates must complete the following health and compliance documents and requirements within 5 days of signing the contract:

• Physical Exam (within 1 year)
• Fit Mask test (annual)
• Negative TB test or Chest X-Ray (within 1 year)
• TB Questionnaire (in lieu of annual TB test)
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Testing (within 30 days of start)
• Seasonal Flu Vaccination or signed declination (during flu season)
• Eye Exam (Snellen, within 1 year)
• COVID vaccination copy preferred but not required

Background & Verification Requirements

• Government issued photo ID required.
• 7-year criminal background check (including felony checks) within 3 months of start.
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter.
• Education verification from primary source required.
• Foreign equivalency evaluation if education obtained internationally.
• Transcript/Diploma required if no education verification on file.
• State license primary source verification within 30 days of start.
• NURSYS verification for LPN/LVN within 30 days of start.
• References: 2 references within 3 years required for Aya candidates.
• Employment application and work history verification required.

State Specific Requirements (New York)

• NYS Nurse Aide Registry (if applicable) within 3 months of start.
• Fingerprinting and disciplinary action search may be required for certain units (Mental Health, Behavioral Health, Home Health Aide, LTC).
• Sex Offender Registry search within 30 days of start.
• NY Medicaid Exclusion List check within 3 months of start and monthly thereafter.

Additional Notes

• Orientation Safety Manual acknowledgment required.
• Medication/Pharmacology competency exams as required by profession.
• Color Blind Testing required once.
• Facility-specific documents and photo for badge required.
• NIH Stroke Certification may be required depending on unit.
• ACLS, PALS, NRP, TNCC, EFM certifications may be required as per unit description.
