Job Description

• Provide nursing care in a rehab/skilled nursing environment.
• Must have at least 2 years of experience in LTC or rehab/skilled nursing settings.
• BLS certification is mandatory.
• Open to first-time travelers and local candidates.

Required Credentials and Compliance

• Licenses & Certifications:

  • Valid LVN/LPN license for Virginia.
  • Current BLS certification (online CPR not accepted).
• Background Checks:

  • 7-year criminal background check within one year of start date.
  • Fingerprinting via Virginia State Police National Criminal Background Check (effective 7/15/2025).
• Health Requirements:

  • Physical exam within the last 12 months.
  • Negative TB test or chest X-ray within 12 months; if positive, clearance letter required.
  • Required vaccinations or proof of immunity for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
    • Seasonal Flu vaccination during flu season (10/1 - 3/31), with medical or religious exemptions accepted upon approval.
    • COVID vaccine series preferred.
  • Drug screening within the last 12 months.
• Documentation:

  • Government-issued photo ID.
  • I9 verification.
  • E-Verify compliance.
  • Photo for facility badge (recent, professional, and meeting specific guidelines).
  • Confidentiality agreement, dress code policy, scrub color agreement, safety brochure, drug-free policy, hand washing hygiene policy, and sworn disclosure forms must be reviewed and signed prior to start.
  • Virginia State Police Check must be completed and cleared prior to start.
  • TB questionnaire updated annually.
• Competency Testing:

  • Medication/pharmacology competency exam as applicable.
  • Pass/fail testing (e.g., EKG/medication) administered during orientation.
• References and History:

  • Two professional references required.
  • Application/resume required.
  • Skills checklist updated annually.

Additional Notes

• Candidates must be at least 21 years old; 25 if transporting patients.
• Minimum 3 years driving experience required if driving is part of the job.
• Facility provides orientation including computer-based learning with multiple attempts allowed.
• All certifications and documentation must be current and compliant with facility and state requirements.
