Job Opportunity: PT Home Health (Therapy/Rehabilitation)
Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 26 weeks (approximately 6 months)
Job Description
- Position for Physical Therapists (PTs) in various California regions including Sacramento, Modesto, and Stockton areas.
- Patient focus: Home Health, Wound Care, Geriatric Home Health Patients.
- Minimum 23-24 patient visits per week required to qualify for a 40-hour guarantee.
- No weekend work required.
- No on-call requirement (on-call is discretionary but not mandatory).
- Locals within a 50-mile radius are accepted.
- Charting must be completed onsite using Kinnser/Wellsky systems; no charting from home.
- Dress code: Any color scrubs and closed-toe shoes.
- Free residential parking available.
- COVID vaccination required (religious exemptions allowed). Flu vaccination declination allowed with proper documentation.
Required Qualifications
- Minimum 2 years overall clinical experience.
- Minimum 1 year experience specifically in Home Health.
- First-time travelers are accepted.
- Must have a valid Physical Therapy state license in hand at time of submission (no pending licenses accepted).
- Basic Life Support (BLS) certification required at time of submission.
- Advanced certifications such as ACLS and PALS preferred but not explicitly required for PT.
Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License/State ID).
- Background checks within 1 year including SS Trace, County searches, Nationwide, and Sex Offender registry.
- Annual EPLS/GSA/SAM and HHS/OIG checks.
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable/infectious diseases.
- Immunizations/vaccinations or documented exemptions required for:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccination, or history of chickenpox)
- Drug screening: 9-panel drug screen within 1 year of start.
- Seasonal flu vaccination or declination form required during flu season (Oct 1 - Mar 31).
- COVID vaccination or approved medical/religious exemption required.
- Auto insurance card required for all candidates.
- Emergency contact form required prior to start.
- References: 2 professional references for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years).
- Annual skills checklist and competency exams as applicable.
Additional Notes
- Contract length is 4-6 months; less experienced clinicians must commit to 6 months (non-negotiable).
- Mileage reimbursed for travel between patients (not to/from home) at IRS standard rates.
- Clinicians are expected to be punctual, patient care focused, self-starters, and diligent in completing charting.
- All required compliance documents must be submitted before starting patient care; compliance processing may take 2-3 weeks.
- Requests for time off (RTO) must be submitted at time of application; no additional RTO accepted after offer.
