Job Opportunity: Registered Nurse (RN) – Home Health (Days)

Location: New Lenox, Illinois
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Provide nursing care through assessment, planning, implementation, and evaluation using the nursing process.
• Supervise CNAs and coordinate care with other healthcare workers.
• Conduct comprehensive initial and ongoing assessments covering physical, psychosocial, environmental, and community resources.
• Initiate, re-evaluate, and modify Plan of Care as needed.
• Establish discharge plans collaboratively with patients, physicians, and other caregivers.
• Complete OASIS data and documentation according to regulatory guidelines.
• Provide patient education tailored to patients, families, and caregivers.
• Promote a clean, safe environment and maintain patient privacy and confidentiality.
• Perform some Telehealth/Virtual visits in addition to home visits.

Required Qualifications

• Specialty: Registered Nurse (RN) with Home Health experience required.
• Strong Med Surg experience with some Home Health experience will be considered.
• Valid Illinois nursing license for RN.
• BLS certification required.
• Valid driver’s license and car insurance required.
• COVID and Flu vaccinations required (declinations will not be approved).
• OASIS and start of care experience strongly preferred.
• Ability to work within a 15-mile territory (may increase if census is low).
• Experience with electronic medical records (EMR) – Matrix system provided via iPad.

Experience

• Minimum years of specialty experience: Not explicitly stated, but Home Health experience is required.
• Prior travel experience: Not required.

Additional Information

• Patient caseload typically 24-26; may decrease if territory expands.
• Orientation: 2 days (1 day in office, 1 day in the field).
