Job Description

  • Position for a Registered/Licensed RN in outpatient services.
  • Workdays are during the day shift.

Required Credentials & Certifications

  • BLS (Basic Life Support) – Required
  • Nursys License Verification – Required for submission
  • Professional State License – Must comply with applicable state executive orders and board guidelines; out-of-state licensure accepted when permitted
  • Catholic Core Competencies – Required
  • Catholic Skills Checklist – Required
  • Catholic Unit Exam – Required
  • Catholic Relias Color Vision – Required
  • Driver's License/State ID – Required
  • Catholic Professional Reference – Required
  • Catholic Employment Verification – Required
  • Catholic Badge Photo – Required
  • Catholic COA (Certificate of Authenticity) – Required
  • Catholic Fit Test – Required

Recommended Certifications (Optional)

  • ACLS (Advanced Cardiovascular Life Support)

Health & Immunization Requirements

  • MMR (Measles, Mumps, Rubella) – Proof of vaccines or titers required
  • Hepatitis B – Proof of vaccines or declination required
  • Influenza Vaccine – Required during October 1 – March 31 (declination accepted)
  • TB Test – One Step TB Skin Test / Tspot / TB QuantiFERON within past 12 months
  • Varicella – Proof of vaccines or titers required
  • Annual TB Questionnaire – Within past 12 months
  • Physical/Health Statement – Physical and vision exam within past 12 months including systems check
  • Catholic Drug Screen – 12 panel (NO THC) within past 12 months

Background & Compliance Checks

  • Catholic Criminal Background Check – 7-year county search, SSN trace, national database within 12 months
  • Catholic National Sex Offender Registry – Within 30 days prior to start
  • Catholic State Sex Offender Registry – Within 30 days prior to start (state of residence)
  • Catholic Webchecks – Within 30 days prior to start

Additional Requirements

  • Catholic Employee Bio Form – Must include full legal name, SSN, address, phone number, and date of birth
  • Catholic Resume – Required for submission

This position requires compliance with all listed certifications, health screenings, and background checks prior to onboarding.
