Job Opportunity: Inpatient Acute Physical Therapist

Location: Tulsa, Oklahoma
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM


Job Description

  • Evaluate and conduct medically prescribed physical therapy treatment programs.
  • Perform initial and ongoing assessments of patient conditions.
  • Establish, revise, and evaluate individualized plans of care involving patients and families.
  • Deliver therapy interventions using standard physical therapy techniques appropriate to patient conditions.
  • Supervise physical therapy assistants, students, and support personnel.
  • Work requires critical thinking, decisive judgment, and ability to work with minimal supervision in a stressful environment.
  • Follow Standard Precautions and use personal protective equipment as required.

Required Qualifications

  • Valid Physical Therapist license in Oklahoma.
  • Physical Therapist with a minimum of 2 years of experience.
  • Physical Therapist experience within the last year.

Required Credentials & Onboarding Documentation

Submission Required Before Assignment:

  • Professional resume/profile.
  • At least two professional references.
  • Valid Driver’s License/State ID (front and back).

Required Prior to or Upon Start:

  • Signed assignment agreement (offer letter, initial confirmation, and extensions).
  • Forward-facing badge photo with white or blue background (no alterations).
  • Health and immunization documentation including:
    • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted).
    • Varicella: Proof of two vaccines, serological immunity, or verified history (no declinations accepted).
    • Hepatitis B: Vaccination series completion or signed declination.
    • Tdap: Valid within 10 years (no declinations accepted).
    • Influenza vaccine (optional; mask required if declined during flu season).
  • Tuberculosis screening:
    • 2-step PPD or IGRA within specified timeframes.
    • If positive, negative chest X-ray within 1 year and annual TB questionnaire.
  • Fit test for respiratory protection (preferred before start, can be completed onsite).
  • Statement of good health or completed physical exam prior to first assignment, updated annually.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and other required screenings within 30 days of start.
  • Employment verification for the past 7 years.
  • Education verification relevant to clinical work within 90 days of start.
  • Skills checklist for specialty completed within one year of start and updated annually.
  • 12-panel drug screen within 30 days prior to start, updated annually.
