Licensed Practical Nurse (LPN) – Assertive Community Treatment

Location: Morgantown, West Virginia
Contract Length: 13 weeks
Shift: Days (Monday to Friday, 8:00 AM – 4:30 PM)
Hours per Week: 40 hours
Work Setting: Community-based program (no on-site or inpatient beds)

Job Description

• Provide one-on-one care to 6-8 clients per day with chronic mental illnesses such as schizophrenia and bipolar disorder.
• Administer injections and deliver medications to patients in the community.
• Conduct home visits and phone calls to support medication adherence.
• Document each patient encounter separately in EPIC, including participation in a daily morning meeting.
• Must drive own vehicle for community visits (mileage reimbursed at IRS rate).

Patient Population

• Individuals with chronic mental illness, primarily schizophrenia and bipolar disorder.

Required Qualifications

• Current LPN/LVN license.
• BLS (Basic Life Support) certification.
• Experience working with individuals with mental illness in community-based programs.
• Ability to perform injections and medication delivery in a community setting.

Schedule & Work Environment

• Monday through Friday, 8:00 AM to 4:30 PM.
• No weekends, holidays, on-call, or float shifts.
• Dress code: Business casual or scrubs (any color).
• Orientation includes shadowing with current ACT RN.

Additional Notes

• Must have a driver’s license with an address located more than 50 miles from the facility.
• Previous permanent candidates must have been off for 2 years before reapplying.
• Previous travelers must have been off for 2 years after completing a full year.
• Submission rules limit candidates to one position at a time within the system.
• Travel teams must be submitted together.
• Only two candidates can be submitted per opening; additional submissions may be held until a spot opens.
