Job Opportunity: LPN/LVN - Rehab

Job Description

• Work in an inpatient rehabilitation setting with a focus on patients recovering from stroke, brain and spinal cord injuries, amputations, complex orthopedic injuries, and other conditions requiring inpatient rehabilitative services.
• The unit functions similarly to a medical-surgical unit and can be high acuity.
• Approximately 70% of patient cases come referred from a Trauma Center.
• Potential to assist with CNA duties as needed for patient stabilization.
• Must have a minimum of 13 weeks of EPIC experience.
• Must complete the full 13-week contract with satisfactory attendance to be eligible for completion bonuses.
• Local candidates are not accepted.

Schedule & Work Details

• Shifts: 12-hour shifts (6:30 AM – 7:00 PM; 6:30 PM – 7:00 AM)
• Weekend Hours: Every other weekend and holiday rotations
• Call Requirements: Yes
• Patient Ratio: 1 nurse to 7 patients
• Number of Beds: 49
• Equipment used includes mobility aids (ceiling lifts, Sara Stedy, Sara Plus, slide boards), vital machines, EKG, blood glucose monitors, and EPIC EHR system.

Required Qualifications

• Valid LPN/LVN license in Wisconsin
• Minimum 1 year of Long Term Care experience within the last year
• Experience in a Rehabilitation setting
• Medical-Surgical experience
• Skilled Nursing experience
• Minimum 13 weeks of experience with EPIC
• Basic Life Support (BLS) certification

Additional Information

• May be required to float to a satellite inpatient rehab facility within 20 minutes in the near future.
• Breaks: Two 15-minute paid breaks and one 30-minute unpaid break per 12-hour shift.
• Staff must clock out for the 30-minute break and cannot stay past shift end without approval.
• Orientation dates available throughout the year.

• Adults requiring inpatient rehabilitation services with high acuity needs.

This position offers the opportunity to work in a specialized inpatient rehab environment with a focus on helping patients achieve their highest level of recovery.
