Contract Length

13 weeks

Schedule

Monday–Friday, 8:00 AM–5:00 PM
Some on-call coverage may be required

Role Overview

Provides skilled nursing care to patients in their homes
Teaches and supervises patients, families, and other care team members
Performs initial and ongoing comprehensive assessments, including OASIS when appropriate
Coordinates care with the interdisciplinary team and communicates changes in patient status
Documents clinical and progress notes and participates in quality improvement activities
May manage palliative and end-of-life care needs

Required Qualifications

Valid RN license in Wisconsin
Associate of Science in Nursing / Associate Degree in Nursing
2 years of experience as a Registered Nurse
Home Health experience within the last year
Home Health experience
Driver's License
Proficiency using a tablet
Crisis intervention experience
Family intervention experience

Additional Details

Typical caseload: 6–8 patients per day
Medicare home health visits
Travel radius: 60 miles from the branch
HCHB preferred
