Days
9:00 AM to 5:00 PM
Arrive at 8:30 AM for huddle prep

Required Qualifications

Valid RN license in New York
2 years of clinic experience
Clinic experience within the last year
BLS
ACLS
PALS
Bachelor of Science in Nursing
Experience working with adult patients
Experience working with pediatric patients
Experience in an outpatient setting
Chronic disease management skills
Ambulatory care experience
Urgent care experience

Preferred Qualifications

2 years of inpatient experience

Role Summary

Provides direct patient care in an ambulatory care setting
Performs patient screening, triage, pain management, falls prevention, discharge planning, and patient education
Coordinates care, including referrals, follow-up, and prior authorization support
Supervises LPNs, medical office assistants, nursing attendants, technicians, and supplemental staff
Participates in quality improvement, clinical audits, outcomes reporting, and patient-centered medical home activities
