- Long Term Care (3 years of experience)
- Long Term Care experience within the last year

- 2 supervisory references from the last 2 years of employment
Work Environment / Assignment Notes
- Rural location with limited housing options and public transportation
- Own transportation is required
- Locals within 50 miles are not accepted
- RTO must be submitted at initial presentation and cannot exceed 5 days
- Holidays are not approved
- Annual seasonal flu vaccine or declination is required
- Scrubs: matching top and bottoms
- Orientation is non-billable and 0 hours
