Registered Nurse (Travel/Contract) - Virginia (Pulaski Area)

Job Overview

• Specialty: Registered Nurse (RN) - Home Health
• Location: Pulaski, Virginia (covering Pulaski and south towards Hillsville and Wytheville)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend Rotation: Start of care visits on Saturday and Sunday every third week as needed
• On-Call: Every third weekend (varies by location and profession)
• Average Patients per Day: 5-8
• Travel: Must be willing to travel up to 75 miles from home clinic; approximately 250-300 miles per week
• EMR System: HomeCare HomeBase (HCHB) and OASIS proficiency required

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Dress code: Scrubs

Experience & Requirements

• Years of Experience: Minimum 1 full year of Adult Home Health experience OR 1 full year Hospice experience (depending on specialty)
• First-timers: Not accepted; must have recent specialty experience
• Certifications Required: CPR and BLS (Basic Life Support)
• Licensure: Must have license in hand; pending licenses not accepted
• Prior Travel Experience: Not explicitly required

Productivity Expectations

• 30 points per week (points assigned based on visits and activities)
• Contractor assigned 5-10 patients per day equating to roughly 6-8 points daily

Additional Requirements & Notes

• Must be proficient with HCHB and have used it within the last 6 months; retraining required if last use was over 6 months ago
• RNs are required to be proficient in OASIS
• Local candidates accepted at the same rate
• Weekend and on-call requirements vary; discuss during interview
• Must complete various compliance, background checks, and health documentation prior to start (including but not limited to physical exam, TB test, drug screening, immunizations, and state-specific background checks)
• Must review and sign multiple facility-specific compliance and policy documents prior to start
• Mileage reimbursed at IRS standard rate (compensated from patient home to patient home; commute to/from first and last patient not billable)

This position is ideal for experienced Registered Nurses with home health or hospice experience who are comfortable with travel within a 75-mile radius and working a flexible schedule including some weekends and on-call rotations.
