Job Opportunity: Physical Therapist (Outpatient)

Location: Del Norte, Colorado
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

Specialty: Therapy/Rehabilitation
Discipline: Physical Therapist (PT)
Shift: Approximately 35-40 hours per week; options include 4x10 or 5x8 shifts
Shift Times: Negotiable, typically between 7 AM - 5 PM with a one-hour lunch
Start Date: ASAP

Required Qualifications

Minimum 1 year of experience in specialty (Physical Therapy)
BLS (Basic Life Support) certification required
Valid Physical Therapist state license for Colorado (primary source verification required)
Unexpired government-issued photo ID (e.g., Driver’s License or State ID)
Background checks including SS Trace, County searches, EPLS/GSA/SAM, and HHS/OIG prior to start
National certification/verification as applicable by profession and state
Physical exam within the last 12 months
Negative TB test or chest X-ray as applicable
Required immunizations/vaccinations: Measles, Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccine (flu vaccine mandatory during flu season; no declinations accepted except documented medical exemptions)
Drug screening within 12 months of start
Completion of required competency exams and skills checklist
Colorado CAPS Release Authorization form and CAPS check results required for all candidates

References: 2 professional references within the last 3 years required
Orientation and safety manual acknowledgment required
