Travel/Contract LVN / LPN - Step Down Unit

Location: Middletown, Ohio
Contract Length: 13 weeks

Job Description

• 3x12 hour shifts
• Floating requirements: Traveler will float at Premier’s discretion, including assignments in specialty clusters such as High Risk Respiratory Unit/COVID.
• If assigned to MVH, floating includes multiple campuses within 15 miles.
• Must be available to work every other weekend and holidays.
• Premier Health may cancel up to 2 shifts per 13-week contract due to low census.
• Agreement may be canceled with 2 weeks' notice without fee or fine.
• Must pass an online EKG Tele Competency Assessment prior to starting.
• No license required to apply, but must obtain the required Ohio LVN/LPN license before contract begins.

Required Qualifications

• Valid LPN / LVN license in Ohio
• Minimum of 1 year of telemetry experience within the last year
• Experience in an inpatient setting
• Basic Life Support (BLS) certification issued by the American Heart Association

Experience

• Specialty: LVN / LPN
• Required years of experience: Not explicitly stated; assume standard LVN/LPN competency expected.

Certifications & Competencies

• Basic Life Support (BLS) - AHA only
• Dysrhythmia/EKG Competency Exam - must be passed within 1 year of start or completed within 72 hours of contract signing (3 attempts allowed)

• Other certifications may be required depending on unit assignment (e.g., ACLS, PALS, NRP, TNCC, EFM) but not specifically required for LVN/LPN step down role.

Background & Health Requirements

• 7-year criminal background check within 12 months prior to start
• E-Verify and I-9 documentation required
• Government issued photo ID required
• Education verification (Primary source)
• Physical exam within 2 years indicating fitness for duty
• Negative TB test or chest x-ray (annual update if past positive)
• Drug screening within 12 months, including marijuana panel
• Seasonal flu vaccination or approved exemption required during flu season
• Respiratory safety clearance within 1 year of start
• References: 2 professional references within 2 years (1 must be manager/supervisor/charge nurse) or employment verification if references unavailable
• Skills checklist within 1 year prior to documentation due date

• Travelers are first to float and may be assigned to various units including high-risk respiratory and COVID units.
• Must be flexible and able to work in multiple locations within the health system.
• Must be available for weekend and holiday shifts every other week.
• Premier Health reserves the right to cancel shifts or contracts with notice.
