Job Opportunity: Registered Nurse (Med Surgical) - Travel/Contract

Required Certifications

BLS (Basic Life Support): AHA provider or AHA RQI only (No other providers accepted)
ACLS (Advanced Cardiac Life Support): Required for this position; AHA provider or AHA RQI only
NIHSS: Preferred but not required

Recommended Certifications (Not Required)

PALS (Pediatric Advanced Life Support)
NRP (Neonatal Resuscitation Procedure)
TNCC (Trauma Nursing Core Course)
EFM (Electronic Fetal Monitoring)

Licensure & Verification

Must have valid RN license verified through the state licensing board within 30 days of assignment start
NURSYS verification required within 30 days of start
Illinois Health Care Worker Registry fingerprinting required if no Illinois state license

Background & Compliance Requirements

7-year background check including aliases, sex offender, national and federal checks, education verification, and international checks if applicable
EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
Education verification required (ADN or BSN minimum)
Fingerprinting required for non-licensed positions (must be done in Illinois)
2 professional references required
Employment verification for last 7 years required
Skills checklist for assigned and float units, updated annually

Health & Immunization Requirements

Physical exam within 1 year, updated annually
Fit mask test within 1 year (specific 3M masks required)
Tetanus/Diphtheria/Tdap vaccination or declination (within 10 years)
Negative TB test or chest X-ray within 1 year (positive TB requires chest X-ray and questionnaire)
Rubeola (Measles), Mumps, Rubella: Proof of immunity by titer or vaccination series
Hepatitis B: Proof of immunity or declination
Varicella: Proof of immunity by titer or vaccination series or declination
Drug testing: 9-panel minimum within 30 days of start, updated annually
Seasonal flu vaccination/documentation required for flu season (Oct 1 - Mar 31), with approved medical or religious exemptions only

Facility Specific Requirements

Passport-style photo for badge (plain background, forward facing, no headgear)
COVID-19 and Influenza exemption forms (religious and medical) must be completed and approved at least one week prior to documentation due date
Compliance with facility-specific background attestation if applicable

Additional Notes

Employment application with last 7 years of work history and explanation of any gaps over 30 days required
All documentation and verifications must be completed timely to avoid delays or removal from schedule
