Job Opportunity: Physical Therapy Assistant (PTA) - Acute Care Hospital
Location: Lafayette, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks
Position Details
- Specialty: Therapy/Rehabilitation
- Shift: Monday to Friday, 5x8 hour shifts
- Weekend Requirement: At least one weekend per month
- Work Hours:
- 8 and 10 hour shifts totaling 40 hours per week, or
- 3 shifts of 12 hours (36 hours/week), or
- 4 shifts of 12 hours (48 hours/week)
- Facility Type: Short-Term Acute Care
- Total Staffed Beds: Approximately 731 beds across units
Experience & Certification Requirements
- Years of Experience Required: Minimum 1 year in specialty
- Prior Travel Experience: Not explicitly required
- Licensure: Must have Indiana state license and required certifications in hand before starting (no pending licenses accepted)
- Certifications Required:
- Basic Life Support (BLS) - AHA or RQI accepted
- Holiday Work: Must work 2 out of 3 major holidays
Additional Requirements
- Background Checks: Comprehensive background checks required within 30-90 days of start, including criminal, education verification, and federal exclusions
- Health Documents:
- Physical exam within 12 months of start
- Fit mask test within 12 months (specific approved respirators required)
- Negative 10-panel drug screen within 12 months
- Immunizations and titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (series, titer, or declination)
- Tetanus/Diphtheria/Tdap (strongly recommended)
- TB test within 1 year of start (PPD, QuantiFERON, or T-SPOT)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/30 - 3/31)
- Competency Exams: Medication/Pharmacology test and unit competency exam required
- References: Minimum 2 recent employment references verified within 2 years, from supervisors or managers
- Application: Must include complete work history with explanations for gaps over 30 days, covering last 7 years
