Job Description

• Taking and monitoring patient vital signs
• Assisting patients with eating, dressing, and bathing
• Administering medication
• Communicating changes in patient behavior or health status to Nurses and Physicians

Required Experience

• 2 professional references required

Health & Immunization Requirements

• Physical Exam within 1 year of start (no annual renewal required)
• Negative TB test or Chest X-Ray within 12 months prior to start (Qgold, Tspot, or 2-step PPD accepted)
• Annual TB Questionnaire completion required
• Vaccination documentation or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (vaccination, titers, or declination)
  • Varicella
• Drug Testing: 9 panel within 1 year of start (instant tests acceptable)
• Seasonal Flu Vaccination or Declination for 2025-26
• COVID-19 Vaccine Booster preferred but not required

Competency Exams

• Competency Exam required for all candidates
• Medication Test/Pharmacology Competency Exam required for LPNs and RNs (effective for new contracts starting 6/25/25; pending/missing does not prevent assignment start)
