Skilled Home Health LVN / LPN

Assignment Overview

• Travel/Contract role in Portland, Oregon
• Skilled Home Health setting
• 8-hour day shifts, 5x8 schedule
• 1:1 patient ratio
• Approximately 30 points per week; up to 6 patients per day
• 13-week contract
• Start date: November 9 or November 16
• Weekend support may be required
• No call requirement
• First-time travelers accepted
• Local clinicians accepted within a 50–75 mile radius at the same rate

Required Qualifications

• Active Oregon and Washington licenses required before start; temporary licenses are not accepted
• At least 1 year of skilled Home Health experience
• BLS
• Reliable personal transportation
• Home Care Home Base (HCHB) experience
• Proof of current auto insurance is required for all Home Health positions

Setting and Work Scope

• Home Health facility type
• Service area may occasionally extend up to approximately 60 miles, or 60–90 minutes of total driving per day
• Facility makes every effort to group patient visits and minimize driving

Documentation and Technology

• Home Care Home Base (HCHB)
• Remote Client for Home Health on a laptop
• Rover phone application

Additional Notes

• Up to 3 days of RTO may be automatically approved
• Blackout periods include Thanksgiving, Christmas, and New Year’s weeks
• Free parking
• Mileage reimbursed at the current IRS standard rate
• Business casual attire; no jeans; scrubs permitted but not required
