Negative TB test (true 2-step PPD, Q-Gold, or T-spot) within one year of start; indeterminate or borderline results require a re-test. If positive, a clear negative chest X-ray within 2 years plus annual TB questionnaire is required
N-95 fit test using 3M 1860 or 1870+ masks only, completed within one year and before start, with documentation stating "pass." PAPR hoods are not available
Negative 10-panel drug screen completed within 60 days of start; pending or instant-test placeholders not accepted
Measles, mumps, and rubella: proof of 2 vaccines (preferred) or positive antibody titer; low or equivocal titers require 2 booster doses

Onsite fit test form due Thursday at 2 PM CST before start date and must be brought on the first day
