Hospice Registered Nurse (Travel/Contract)

Location: Leander, Texas
Contract Length: 13 weeks

Job Overview

• Provide skilled nursing care to hospice patients in their homes.
• Responsible for patient admissions, comprehensive assessments, and interdisciplinary care coordination.
• Develop and oversee individualized Plans of Care in collaboration with certifying providers.
• Average caseload: 4-6 patients per day.
• Charting required using Homecare Homebase (HCHB) and OASIS, completed while at patient homes.
• Must participate in an on-call rotation (one night per week, details discussed during interview).
• Work schedule: Monday-Friday, 8:00 AM to 5:00 PM, with weekends as needed.
• Must be willing to travel up to 50 miles from home base to see patients.

Required Qualifications

• Minimum 2 years clinical experience overall, with at least 1 year in hospice nursing.
• Valid Registered Nurse (RN) license in hand at start (no pending licenses accepted).
• BLS certification required.
• Experience with HCHB and OASIS charting systems preferred (1 year preferred).
• Non-smokers preferred due to patient sensitivity (patients with COPD).
• Must be able to complete admissions and see patients the same day.
• Must be open to on-call duties as part of the schedule.

Work Environment & Expectations

• In-home patient care with a geriatric population.
• Interdisciplinary care coordination and plan of care development.
• Dress code: Scrubs, any color.
• Mileage reimbursed at IRS standard rates.
• Locals within a 50-mile radius are accepted.

Compliance & Documentation Requirements

• Current Driver’s License/State ID required.
• Background checks within 1 year, including 7 years of residency/work history, SS trace, county searches, nationwide checks, sex offender registry, FACIS Level 1, and state driver’s record.
• Annual EPLS/GSA/SAM and HHS/OIG checks required.
• Certifications:

  • BLS (AHA or American Red Cross, RQI accepted)
  • ACLS (AHA or American Red Cross, RQI accepted)
  • PALS (AHA or ARC only) if applicable
• Orientation Safety Manual acknowledgment required.
• Competency exams including medication/pharmacology, infection control, and unit-specific exams required.
• Personal or auto insurance documentation required.
• Physical exam within 12 months confirming good health.
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire.
• Proof of immunity or vaccination for Measles, Mumps, Rubella, Hepatitis B (or declination), Varicella, and seasonal flu vaccination or declination during flu season.
• 9-panel drug screen within 12 months.
• References:

  • 2 professional references for travel, allied, or per diem acute care roles.
  • 1 professional reference for per diem non-acute care roles.
• Employment application and annual skills checklist required.

Additional Notes

• First-time travel nurses accepted.
• Must be able to hit 30 points per week (equivalent to approximately 40 hours/week) to be considered full-time. Overtime requires pre-approval.
• On-call rotation includes one night per week.
• Non-smoking policy due to patient population sensitivity.

This position offers the opportunity to work in a home health hospice setting, providing critical care coordination and skilled nursing to a diverse geriatric patient population in a travel nurse capacity.
