Job Summary

Seeking a Physical Therapist for a home health travel contract position. The role involves providing therapy services primarily to geriatric patients within a 60-mile radius of the office hub. This position requires flexibility with travel territory and timely documentation.

Required Qualifications

  • Valid Physical Therapist license in Washington
  • Minimum 1 year of recent Physical Therapist experience within the last year
  • Completion of at least one travel assignment
  • Basic Life Support (BLS) certification
  • Experience with OASIS documentation

Background and Compliance Requirements

  • Background checks including SSN trace, county searches, nationwide sex offender, motor vehicle records, education and work history verification (within 90 days of start, updated annually)
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
  • State-specific criminal history and employability status checks as applicable
  • Verification of highest degree related to profession
  • Drug screening (9 panel) within last 12 months
  • Physical exam within last 12 months
  • Fit mask test (N95 accepted)
  • Negative TB test or chest X-ray within last year (or TB questionnaire if applicable)
  • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
  • Seasonal flu vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required

Additional Requirements

  • 2 professional references within 2 years of start (travel, allied, per diem acute care)
  • Full 7 years of work history documented with no gaps
  • Annual skills checklist and competency exams as applicable
  • Proof of current auto insurance required due to driving responsibilities

Work Environment

  • Home health setting with travel within a 60-mile radius
  • Scrubs dress code, no jeans allowed
  • Use of provided iPad for documentation
  • On-call every 6th weekend
