Job Opportunity: Licensed Masters of Social Worker (LMSW)

Position Details

• Specialty: Hospice Social Services (experience in Hospice required)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Schedule: Monday to Friday, approximately 24-25 hours per week (typically 3 shifts of 8 hours, 8:00 AM to 1:00 PM)
• Average Patients per Day: 5-7 (geriatric cases)
• On-Call: No
• Weekends: No
• Dress Code: Scrubs (any color), no jeans
• Territory: Up to a 60-mile radius from the office; flexibility with territory expected
• Locals accepted at travel rate
• Device Provided: iPad (cell phone use not allowed)
• Mileage reimbursed at IRS standard rate

Required Qualifications

• Minimum 1 year recent experience in Hospice Social Services
• Experience with HCHB Electronic Medical Record (EMR) system
• Current license to practice LMSW in the state of New Hampshire
• Basic Life Support (BLS) certification (AHA, ARC, or Military accepted)
• Timely documentation and communication skills

Candidate Requirements

• First-time travelers NOT accepted (prior travel experience required)
• Must be flexible with travel territory within a 60-mile radius
• Proof of current auto insurance required if job responsibilities include driving

Compliance and Background Checks

• Comprehensive background checks required within 90 days of start, including but not limited to:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification (7 years)
  • State-specific searches as applicable (e.g., New Hampshire State Police Search)
• Fingerprinting may be required depending on placement location and must be current (within 12 months)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start

Certifications and Competencies

• BLS certification required
• Medication Test/Pharmacology Competency Exam as applicable
• Competency exams relevant to profession/specialty

Health and Immunization Requirements

• Physical exam within the last 12 months (physician statement)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire required
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screening: 9-panel drug test within 12 months
• Seasonal flu vaccination or facility-specific declination form during flu season
• COVID vaccination or facility-specific declination

Documentation and History

• Employment application with full 7 years of work history, no gaps
• Two professional references within 2 years of start date
• Annual skills checklist required
• Facility-specific documents such as Acceptable Use Policy and Agency Background Attestation must be completed prior to start

This position is ideal for experienced hospice social workers with recent hospice experience, who are comfortable working in a travel contract role with a flexible territory and are proficient in HCHB EMR documentation.
