Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Must be willing to travel up to 75 miles from the home clinic.
• Charting is done using HomeCare HomeBase (HCHB); proficiency required within the last 6 months or retraining will be required.
• Typical schedule is Monday through Friday, 8 AM to 5 PM.
• Weekend and on-call requirements vary; discuss during interview.
• Local candidates accepted.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience or Hospice experience (depending on specialty).
• Recent specialty experience required; first-time travelers are not accepted.
• Current LVN/LPN license in hand; pending licenses are not accepted.
• CPR and BLS certification required.
• Proficiency with HomeCare HomeBase (HCHB) EMR system.
• Must have reliable transportation and valid driver’s license.
• Ability to travel within a 75-mile radius of the home clinic.

Schedule & Workload

• Standard schedule: Monday to Friday, 8 AM to 5 PM.
• No guaranteed hours; patient census may vary.
• Average productivity expectation: approximately 32 points per week (points relate to visits and documentation).
• Some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8.

Compliance & Documentation Requirements

• Must complete standard background checks including 7-year history, sex offender, and state-specific registry checks.
• Drug screening (10-panel) required.
• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
• Current BLS certification.
• Must review and sign various compliance documents including confidentiality, HIPAA, privacy policies, and home health core policies.
• Personal or auto insurance required for home health placements; policy must be current and list candidate as insured or driver.
• References: Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.
