Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Travel/Contract

Location: Gainesville, Georgia
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
  • Conduct home visits and periodic office meetings; reliable transportation and current driver's license required.
  • Average patient load: 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Visits occur both in patient homes and care facilities.
  • Schedule typically Monday to Friday, 8 AM to 5 PM; some locations may have 4x10 shifts (Friday-Monday).
  • On-call required: 1 day per week plus weekend call rotation every 3rd weekend.
  • Must be willing to travel up to 75 miles from home clinic.

Required Qualifications

  • Valid LPN / LVN license in Georgia
  • Minimum 1 year of recent Home Health experience, including experience with adult patients
  • Minimum 1 year of recent Hospice experience
  • BLS certification issued by American Heart Association or American Red Cross
  • Experience working in a Home Health setting
  • Proficiency with HomeCare HomeBase EMR system

Schedule & Workload

  • Typical schedule: Monday through Friday, 8 AM to 5 PM.
  • Patient visits: 5-8 per day, equating to approximately 6-8 points daily (productivity measured in points).
  • Weekly productivity expectations: 32-35 units.
  • On-call: 1 day per week plus every 3rd weekend rotation.
  • Travel: Approximately 30-40 miles per day within a 75-mile radius.

Additional Requirements & Compliance

  • Must complete a standard background check including 7 years of history, sex offender search, and state-specific screenings.
  • Drug screening: 10-panel required.
  • Health documents required: Physical exam within 12 months, negative TB test or chest X-ray within 12 months, immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella, and COVID vaccination or declination.
  • Must review and sign various compliance documents including confidentiality, HIPAA, privacy policies, and home health scenarios prior to start.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
  • Locals accepted at the same rate as travelers.
