Physical Therapist - Home Health

Location: Palm Springs, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

Schedule: Monday to Friday, 8:30 AM to 5:00 PM (30-minute lunch)
No on-call or weekend requirements
Average caseload: 5-7 geriatric home health patients per day
Territory coverage: 50-mile radius from office/hub
Mileage reimbursed at IRS standard rate between patient homes
Electronic Medical Record (EMR) system: HCHB (experience required)
OASIS experience required

Required Qualifications

Valid Physical Therapist license in California
Minimum 1 year of Physical Therapist experience within the last year
Experience in a Home Health setting
Basic Life Support (BLS) certification

Certifications & Compliance

Current BLS certification (preferably from American Heart Association or American Red Cross)
Physical exam within 1 year of start date
9-panel drug screen within 1 year of start date (instant drug screen acceptable)
Seasonal flu vaccination or documented declination required during flu season (Oct 1 - Mar 31)
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chickenpox)
Annual competency exams and medication/pharmacology competency as applicable
Background checks including 7-year work/living history, sex offender registry, and federal exclusions (updated annually)
Emergency contact form and government-issued photo ID required prior to start
Professional references: 2 for travel/allied/per diem acute care roles (within 3 years)

Additional Information

Locals accepted at travel rate
No weekend or on-call duties
Must meet or exceed required competency exam scores if not completed through Relias

This position is ideal for experienced home health physical therapists licensed in California who have prior home health experience and are comfortable working independently in a travel contract role.
