Physical Therapist - Home Health (Travel/Contract)
Location: San Jose, California
Contract Length: 16 weeks (approximately 4 months)
Job Overview
- Specialty: Therapy/Rehabilitation (Physical Therapist)
- Employment Type: Travel/Contract
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
- Weekly Hours: 40 hours guaranteed
- Patient Load: Minimum 23-24 visits per week required
- Patient Types: Home Health, Wound Care, Geriatric Home Health Patients
- Weekend Work: Not required
- On-Call: Not required (discretionary but not mandatory)
- Locals Accepted: Yes, within a 50-mile radius
- Mileage Reimbursement: IRS standard from first to last patient visit (not from/to home)
- Dress Code: Any color scrubs, closed-toe shoes
- Charting System: Kinnser/Wellsky; charting must be completed onsite, no remote charting allowed
- Parking: Free residential parking available
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Background checks within 1 year, including SS Trace, County searches (7 years), Nationwide, Sex Offender, EPLS/GSA/SAM, and HHS/OIG checks (annual updates required)
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable/infectious diseases
- Immunizations and titers as applicable:
- COVID-19 vaccine or approved medical/religious exemption
- Flu vaccine or facility-specific declination form during flu season (Oct 1 - Mar 31)
- Tetanus/Diphtheria/Tdap vaccine within 10 years or approved exemption
- Measles, Mumps, Rubella (MMR) proof or declination as per birth year guidelines
- Hepatitis B titer, vaccine series, or declination
- Varicella titer, vaccine, history of chickenpox, or declination
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 5 years and annual TB questionnaire
- 9-panel drug screen within 1 year (instant drug screen acceptable)
- Auto insurance card (if applicable)
- Emergency contact form prior to start
- Professional references:
- 2 for travel, allied, or per diem acute care
- 1 for per diem non-acute care (not required for non-clinical roles)
- Annual skills checklist and competency exams as applicable
Additional Notes
- Contract length for less experienced clinicians (under 2 years) is non-negotiably 6 months
- Territory and county coverage details will be discussed during the interview; may include surrounding counties
- Valued clinician qualities: patient-focused care, punctuality, self-starter attitude, and timely completion of charting (tablet provided onsite)
