Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from the home clinic; mileage reimbursed at IRS standard rate (travel between patient homes is compensated, commute to/from first and last patient is not).
• Schedule typically Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
• Some positions may have alternate schedules such as 4x10 shifts (Friday-Monday).
• Use of electronic medical records system HomeCare HomeBase (HCHB) required; proficiency in OASIS required for RNs.
• Dress code: Scrubs.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience (depending on specialty).
• Recent specialty experience required; first-time travelers not accepted.
• Current Registered Nurse (RN) license in hand; pending licenses not accepted.
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; if not current, retraining required.
• Proficiency in OASIS documentation required.
• Must be willing to travel within the assigned territory.

Additional Requirements

• Background checks including 7-year history, county/state/nationwide searches, sex offender registry, and drug screening.
• Education verification required.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella.
• COVID-19 vaccination or declination documentation required.
• Flu vaccination or declination documentation required.
• Various state-specific background and fingerprinting requirements depending on assignment location.
• Must complete required compliance, confidentiality, privacy, and policy acknowledgments prior to start.
• Must complete Home Health Core Care Policy Quiz and review Home Health Scenarios prior to start.
• Two professional references required.
• Verification of work history for the last 7 years or at least two most recent employers.

On-Call and Weekend Requirements

• Weekend and on-call requirements vary by location and will be discussed during the interview.
• No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.

Travel and Local Candidates

• Local candidates accepted at the same rate.
• Must be willing to travel within the designated territory (up to 75 miles radius).

This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable working independently in patients' homes and managing a moderate patient load with travel requirements.
