  • Contract Length: 13 weeks

Schedule

  • Shift: Day shift, 5 days per week, 8 hours per day (8:00 AM – 4:30 PM)
  • Weekend scheduling varies depending on patient visits
  • Territory scheduling aims to limit drive time between patients

Position Details

  • Number of openings: 2
  • Specialty: Home Health outpatient Physical Therapist Assistant (PTA)
  • Productivity requirement: 70% productivity (units billed correspond to hours paid; up to 40 hours/week)

Required Qualifications

  • Valid Physical Therapy Assistant license in Colorado
  • Minimum 1 year of Physical Therapy Assistant experience within the last year
  • Basic Life Support (BLS) certification

Required Documentation & Compliance

  • Background checks within 1 year, including SS trace, county searches (7 years), nationwide, sex offender, and FACIS Level 1M; updated annually
  • EPLS/GSA/SAM and HHS/OIG checks required annually
  • Physical exam within 1 year of start date
  • Negative TB test or chest X-ray within required timeframes
  • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (vaccines, titers, or declination accepted)
  • Negative 9-panel drug screen within 1 year of start
  • Seasonal flu vaccination or declination required during flu season (Oct 1 – Mar 31)
  • References: 2 professional references for travel/allied/per diem acute care or 1 for per diem non-acute care within 3 years
  • Annual competency exams and medication/pharmacology competency as applicable
  • Facility-specific confidentiality agreement must be reviewed and signed
