Job Opportunity: COTA Home Health (Travel/Contract)

Location: Everett, Washington
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (Day shift, 5x8 hours)
Contract Length: 13 weeks

Job Overview

• Specialty: Certified Occupational Therapy Assistant (COTA) in Home Health
• Average Patients per Day: 5-7 (to be confirmed during interview)
• Types of Cases: Geriatric
• Guaranteed Hours: 40 hours per week
• On-call Requirement: Yes, every 6th weekend
• Territory includes multiple nearby cities; flexibility with territory is expected
• First-time travelers are not accepted
• Locals are not accepted
• Dress Code: Scrubs of any color; jeans are not allowed
• Documentation and communication must be timely
• iPad provided for work; personal cell phones are not allowed
• Mileage reimbursed at IRS standard rate

Required Qualifications

• Valid Certified Occupational Therapy Assistant license in Washington
• Minimum 1 year of Certified Occupational Therapy Assistant experience within the last year
• Completion of at least one travel assignment
• Basic Life Support (BLS) certification (AHA, ARC, Military accepted)
• Experience with OASIS documentation
• Must be flexible with travel territory

Compliance and Background Requirements

• Background checks must be current within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education Verification (highest degree related to profession)
  • Work History Verification (7 years, with attempts to verify if needed)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting requirements vary by state; for Washington, fingerprints required if candidate has not lived in WA for the past 3 years (completed on-site)
• State-specific criminal history and employability status checks required within 90 days of start

Health and Safety Requirements

• Physical exam within the last 12 months (not required annually)
• Fit test for N95 mask (no specific model required)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if previously positive; annual TB questionnaire required
• Immunizations or proof of immunity required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required
• 9-panel drug screen within 12 months (instant acceptable)

Documentation and Other Requirements

• Two professional references within 2 years of start (travel, allied, per diem acute care)
• Employment application with full 7 years of work history, no gaps
• Annual skills checklist and competency exams as applicable
• Proof of current auto insurance required due to driving responsibilities
• Completion of facility-specific policies and attestations prior to start (e.g., Acceptable Use Policy, Agency Background Attestation)

• This is a travel/contract position; prior travel experience is not explicitly required but first-time travelers are not accepted
