Work Schedule

  • Day shift, Monday through Friday
  • 8 hours per day (8:00 AM – 5:00 PM)
  • Guaranteed 40 hours per week
  • Occasional weekend work may be required

Caseload

  • Average 5-7 patients per day
  • Primarily geriatric cases

Territory

  • Up to a 60-mile radius from the office hub (typically about 30 miles)
  • Flexibility with territory coverage expected

Required Qualifications

  • Valid Physical Therapist license in Montana
  • Minimum 1 year of Physical Therapist experience within the last year
  • Basic Life Support (BLS) certification
  • Experience with Homecare Homebase electronic medical record system

Equipment Provided

  • iPad provided for work use
  • Cell phone use not permitted for work purposes

Mileage

  • Mileage reimbursed at IRS standard rate

Background and Compliance Requirements

  • Comprehensive background checks required within 90 days of start, including but not limited to:
    • SSN trace
    • County searches for last 7 years
    • FACIS Level 1
    • Nationwide sex offender registry
    • Motor vehicle records
    • Education and work history verification
    • State-specific searches as applicable
  • Fingerprinting may be required depending on placement state
  • Employment application with full 7 years of work history, no gaps
  • Two professional references required within 2 years of start
  • Annual competency exams and skills checklists as applicable
  • Signed orientation safety manual acknowledgment (updated annually)

Health and Immunization Requirements

  • Physical exam within the last 12 months (not required annually)
  • Negative TB test or chest X-ray per guidelines
  • Annual TB questionnaire required
  • Immunizations or proof of immunity for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • 9-panel drug screen within 12 months of start (instant acceptable)
  • Seasonal flu vaccination or facility-specific declination form during flu season
  • COVID vaccination or facility-specific declination

Facility-Specific Documents

  • Proof of current auto insurance required (due to driving responsibilities)
  • Completion of facility’s Acceptable Use Policy prior to start
  • Facility-specific flu and COVID declination forms if applicable

  • First-time travelers are accepted
  • Local candidates accepted at travel rates
