Required Qualifications

Valid Hospital Pharmacist license in Virginia
Hospital Pharmacist (3 years of experience)
Hospital Pharmacist experience within the last year
BLS - Basic Life Support issued by American Heart Association
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
Experience in a(n) Acute Care setting
Experience in a(n) Inpatient setting
Experience with Epic

Preferred Qualifications

RPh
Pharmacy residency from an accredited ASHP institution
Intravenous experience
Pharmacotherapy management experience
Experience in a computerized hospital pharmacy setting
Ability to work independently
Strong interpersonal and communication skills

Role Focus

Complex medication reconciliation
Order recommendations and order changes on behalf of the hospitalist group
Independent practice in a pharmacist-as-practitioner model
