Inpatient Pharmacist (40 hrs, Days/Eves Rotate)

Location: Salem, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

• Work schedule: Rotating shifts between 7:00 AM - 3:30 PM and 2:30 PM - 11:00 PM
• Unit: Inpatient Pharmacy
• Patient population: Newborn through geriatric, including medical and behavioral health patients
• Responsibilities:

  • Review and interpret medication orders for safety and efficacy
  • Oversee pharmacy technician activities such as medication dispensing and IV compounding
  • Maintain strong attention to detail
• Systems used: Epic EMR, Omnicell

Required Qualifications

• Valid Pharmacist license in Massachusetts
• Pharmacy degree
• Minimum 1 year of pharmacist experience within the last year
• Experience in an inpatient setting

Onboarding & Compliance Requirements

• Professional:

  • Resume including past 7 years of work experience with city and state
  • Two professional references within last 4 years (minimum one supervisor-level)
  • Skills checklist completed within 1 year
  • Criminal background check within 45 days prior to start, including Massachusetts CORI and national sex offender registry checks
  • Social Security Number trace covering last 7 years
  • Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists
  • Education verification of highest degree achieved
  • Confirmation of Assignment/POI form upon acceptance and for extensions
• Health:

  • Documentation of MMR immunity (vaccines or blood test)
  • TB screening within 3 months prior to start (skin test or blood test), updated if break >6 months
  • Physician health statement within 1 year of start, updated if break >6 months
  • Hepatitis B vaccination or immunity strongly preferred
  • Tdap vaccine within last 10 years preferred
• Additional:

  • Name change documentation if legal name differs from credential documentation
