Employment Type: Long-Term Assignment (LTA)

Position as a Speech Language Pathologist in an outpatient setting.
Shared housing is available (no guests or pets allowed). Must have own transportation.
Candidate must have received the flu vaccine.

Valid Speech Language Pathologist license in Massachusetts
Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross

Positive TB test requires documentation of a negative chest x-ray after the positive test.
Name change documentation required if legal name differs from credential or profile documentation.
TB symptom survey required if positive TB test, updated annually.
Candidate submission coversheet required for initial assignment submissions.

Must have own transportation to and from work.
No guests or pets allowed in shared housing.
All required documentation must be current and updated as specified to maintain compliance.
