Physical Therapist Outpatient - Nantucket, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Primarily outpatient work (90%) with some acute care (10%); outpatient-only candidates will be considered.
- Flexible scheduling options:
- 10-hour shifts during the week
- Choice of days
- Half-day shift on Saturday
- Shifts start between 7:00 AM and 8:30 AM.
- Start dates available in February or March.
- Shared housing provided at no cost.
Requirements
Licensure & Certifications
- Valid Massachusetts Physical Therapist license (MA Conditional Licenses not accepted).
- Basic Life Support (BLS) certification (AHA preferred; Red Cross accepted).
- Completion of FEMA ICS 100 Training (mandatory, no exceptions).
- Professional license and certification primary source verification required within 30 days of start.
Health & Immunizations
- Documentation of:
- Two measles, two mumps, and one rubella vaccine OR two MMR vaccines, or proof of immunity via blood test.
- Influenza vaccine during flu season (Sept 15 - April 30); no exemptions allowed.
- TB screening within 3 months of start (skin test or blood test) and updated if >6 month break.
- Physician health statement within 365 days of start, updated if >6 month break.
- Strongly preferred: Hepatitis B vaccination or proof of immunity.
- Tdap vaccine within 10 years preferred.
- Fit test for N95 respirator within 365 days of start, updated annually; no religious exemptions.
- Respirator Medical Clearance attestation if fit test required.
- If positive TB test, documentation of negative chest X-ray required.
Background & Compliance Checks
- Criminal background check within 45 days prior to start, including international searches if applicable.
- Massachusetts CORI background check within 30 days prior to start.
- National sex offender registry check within 30 days prior to start, updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually.
- Social Security Number trace as part of criminal background check.
- Education verification of highest degree achieved.
- Submission of 2 recent professional references (minimum 1 supervisor or above).
- Submission of resume including past 7 years of work experience with city and state.
- Completion of skills checklist and candidate submission coversheet required.
Additional Information
- Housing agreement required upon hire if staying in provided housing.
- Confirmation of assignment form required upon acceptance and for any extensions.
- TB symptom survey required if positive PPD.
- No onsite fit testing available at most locations; updates possible on Martha’s Vineyard & Nantucket if assignment extended.
