Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back copies with expiration date.
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Must provide scanned front and back copies with expiration date.
• NIHSS (National Institutes of Health Stroke Scale): Required at onboarding (optional for submission).
• License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required. For RNs and LPNs, state verification preferred (e.g., NURSYS).

Health & Immunization Requirements

• Fit Test: Required at onboarding.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferably within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, methadone. No THC allowed. Must be resulted before start with MRO signature.
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL). Valid through October 2026.
• MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations with dates or IgG titer lab report required.
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Declination not accepted in CA or IL. Valid for flu season 10/1/2024 - 4/30/2025 (and subsequent season).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for certain states; otherwise within 5 years.
• Varicella: Proof of 1 vaccination with date or IgG titer lab report required.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction; agency form acceptable.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional & Compliance Requirements

• Trinity Profile: Resume, skills checklist (within one year), and 2 professional references required for submission.
• Trinity Code of Conduct Form: Signed form required at onboarding.
• Trinity I9: Standard I9 completed by agency required at onboarding.
• Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information required at onboarding.
• Integrity and Audit Services Form: Signed within days prior to start.
• Non-Employee Upload Form: Completed and uploaded pages 1-3 and 28 required.
• Specialty Specific Competency Testing: Copy of specialty exam completed within one year required.
• Conflict of Interest Disclosure Form: Signed form required at onboarding.
• Criminal Background Check: Federal, nationwide, county criminal check for last 7 years plus SSN verification. Must be dated within 30 days of start. Includes employment verifications for last 7 years.
• Education Verification: Third-party screening report verifying highest academic education required.
• EPLS (Excluded Parties List System): Scanned report dated within past 30 days required.
• GSA/OIG Exclusion Check: Scanned report dated within past 30 days required.
• Joint Commission Testing: CORE exam within one year required.
• NSO (National Sex Offender) Check: Dated within 30 days of start, all aliases checked. MI requires additional statewide check.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.
