Physical Therapist - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 17 weeks)


Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice. Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.


Required Qualifications

  • Valid Physical Therapist license in Massachusetts
  • Minimum 1 year of Physical Therapist experience, including experience within the last year
  • Experience in a Home Care setting

Onboarding & Compliance Requirements

  • Submission of a professional resume including the past 7 years of work experience with city and state details.
  • Two professional references within the last 4 years, including at least one supervisor or higher.
  • Completion of a skills checklist within 1 year of submission.
  • Background checks including criminal, CORI (Massachusetts criminal history), SSN trace, national sex offender registry, and exclusion checks.
  • Verification of highest level of education achieved.
  • Submission of a color photo meeting specified criteria (recent, plain background, no filters).
  • Documentation of immunizations and health screenings:
    • MMR (measles, mumps, rubella) vaccination or proof of immunity.
    • TB test within 3 months of start date (skin test or blood test) and updated if there is a break in service longer than 6 months.
    • Physician health statement within 1 year of start date and updated if >6 month break.
    • Fit test for N95 respirator within 1 year of start date and updated annually (no exemptions allowed).
  • Respirator Medical Clearance attestation if fit test is required.
  • Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees).
  • Confirmation of assignment form required for all initial assignments and extensions.

Additional Notes

  • Weekend rotations are infrequent (once every 6 weeks).
  • Prior travel experience is not explicitly required but ability to travel to patient homes is mandatory.
  • MA conditional licenses are not accepted.
  • All documentation and background checks must be current and updated if there is a break in service longer than 6 months.
