Physical Therapist - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM


Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range spans from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to assess needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice. Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.


Required Qualifications

  • Valid Physical Therapist license in Massachusetts.
  • Minimum of 1 year of Physical Therapist experience within the last year.
  • Experience in a Home Care setting.
  • Must be able to travel to patient locations as assigned.

Required Documentation & Compliance (Must be completed prior to or at onboarding)

  • Professional license verification and primary source validation within 30 days of start.
  • Criminal background check, including Massachusetts CORI and national sex offender registry, within 45 days prior to start and updated as required.
  • Motor Vehicle Record (MVR) check within 45 days prior to start (required for all home care positions).
  • Resume including past 7 years of work experience with city and state details.
  • Two professional references within 4 years, including at least one supervisor or higher in the relevant skillset.
  • Skills checklist completed within 1 year of submission.
  • Photo ID badge (color photo taken within past 6 months, plain background, no filters or eyeglasses).
  • Education verification of highest degree achieved.
  • Respirator fit test (N95) within 365 days of start date, updated annually; no exemptions allowed.
  • Respirator Medical Clearance attestation if fit test required.
  • Tuberculosis (TB) screening: documentation of TB skin test or blood test within 3 months of start, updated if break >6 months.
  • Health statement signed by MD/DO/NP/PA within 365 days of start, updated if break >6 months.
  • Measles, mumps, rubella (MMR) vaccination documentation or proof of immunity.
  • Criminal and exclusion checks including OIG, GSA, MassHealth, and NH Medicaid exclusion lists within 30 days of start and annually.
  • Social Security Number trace as part of background check.
  • Confirmation of assignment form upon acceptance and for any extensions.

Preferred (Optional)

  • Hepatitis B vaccination or proof of immunity (or declination).
  • Tdap vaccination within the last 10 years.
  • Documentation of chest x-ray if positive TB test.

Additional Notes

  • No religious or medical exemptions allowed for N95 fit testing.
  • Fit testing cannot be performed onsite; must be completed prior to start.
  • Weekend rotations are infrequent (once every 6 weeks).
  • Travel to patient homes is a required part of the role.
