Job Opportunity: LPN, VNA Home Care

Location

Northampton, Massachusetts

Employment Type

Long-Term Assignment (approximately 13 weeks)

Schedule

Monday to Friday, 8:00 AM – 4:30 PM
One weekend shift required every 4 weeks (1 weekend rotation per month)

Job Description

Provide skilled nursing care in patients' homes, performing approximately 6 patient visits per day.
Work collaboratively within a multidisciplinary team including RNs, Physical Therapists, Occupational Therapists, and Home Health Aides.
Deliver care as delegated by the RN Case Manager or Clinical Team Manager.
Must have prior home care experience.
Practice in accordance with the State Nurse Practice Act, professional standards, and applicable laws.

Required Qualifications

Valid LPN/LVN license in Massachusetts.
Minimum of 1 year of home health/home care experience, with experience within the last year.
Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
