Required Certifications and Credentials

• BLS (American Heart Association)
• Current professional license with primary source verification within 30 days prior to assignment
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) completed within one year, updated yearly, minimum 80% score
• Ascension Surgical Technologist Contract Worker Onboarding Education Attestation
• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Fit Test: Required for all travelers, preferred prior to start, updated annually
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31), declinations accepted with mask requirement
• TB Screening: Positive IGRA or PPD results, negative chest X-ray within one year, TB questionnaire completed and updated annually
• Health Statement: Completed physical exam or statement of good health prior to first assignment, updated annually
• Driver’s License/State ID: Valid, must match address requirements for compact license states
• 2 Step PPD or IGRA within required timeframes
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap: Valid within 10 years, no declinations accepted
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually
• Ascension Skills Checklist: Clinical skills checklist completed within one year, updated annually

Professional Documentation

• Resume/Profile submission required
• At least two professional references prior to first assignment
• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender, and other checks within 30 days of assignment, updated annually
• EPLS check within 90 days of start, rerun if break in service > 90 days
• Education Verification within 90 days of start, updated if break in service > 90 days
• Confirmation of Assignment documentation required upon initial assignment and updates for extensions
• Employment Verification for past seven years, updated annually or if break in service > 90 days
• Badge Photo: Forward-facing headshot with white or blue background, no alterations or filters
