Job Opportunity: X-Ray Technologist

Location: Mishawaka, Indiana
Employment Type: Long-Term Assignment (LTA)
Schedule: Monday through Friday, 8:00 AM – 4:30 PM
Contract Length: 13 weeks

Required Certifications & Licenses

• ARRT Certification (required for submission)
• BLS Certification (required for submission)
• BLS Certification (American Heart Association, required for onboarding)
• License/Certificate Verification (must be dated within 30 days of start)

Health & Immunization Requirements

• Fit Test
• Annual TB Questionnaire
• Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or approved declination/exemption; declination not accepted in CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start)
• Influenza Vaccine (proof or declination/exemption for 10/1/2024–4/30/2025 and 10/1/2025–4/30/2026; declination not accepted in CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per state-specific guidelines)
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity Color Vision test
• Trinity Latex Allergy Questionnaire
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)

Professional & Compliance Requirements

• Trinity Profile (resume, skills checklist within a year, 2 professional references) – required for submission
• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality & System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission Testing (CORE Exam within the last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years, SSN verification, dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS Report (dated within 30 days)
• GSA/OIG Exclusion Report (dated within 30 days)
• NSO Report (dated within 30 days, includes all aliases and locations)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• OFAC Sanctions Search (completed within 30 days of start)
• Orientation Guide (completion required)
