Physical exam within 1 year of start; must be completed within 72 hours of contract signing
Fit mask test within 1 year; updated annually (specific mask types required)

Negative TB test or chest X-ray within 1 year (chest X-ray within 2 years if positive TB test)
Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps

- Varicella
Drug testing: Negative 10-panel drug screen within 30 days of start; must meet facility requirements and be completed within 72 hours of contract signing
Seasonal flu vaccination or declination required during flu season (October-April)
