Job Opportunity: Physical Therapist - Outpatient

Location: Sunnyside, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday (5x8 hours), 8:00 AM to 5:00 PM
Hours per Week: 40 hours

Job Description

• Outpatient Physical Therapy position
• Provide therapy and rehabilitation services in an outpatient setting

Required Qualifications

• Valid Physical Therapist license in Washington
• Minimum of 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification

Background and Compliance Requirements

• 10-year criminal background check including SSN trace, alias/name changes, statewide and national checks, sex offender registry, and education verification
• Washington State Patrol (WSP/WATCH) search required within 30 days of start
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Education verification required for all candidates
• Employment application and 3 recent employment references required

Certifications

• Basic Life Support (BLS) certification (AHA or ARC accepted)

Health and Immunization Requirements

• Physical exam with physician-signed health statement within 1 year of start
• Fit testing for N95 mask performed onsite (required for certain locations)
• Tetanus/Diphtheria/Tdap vaccine or signed declination
• Negative TB test (QGold or T-SPOT) within 30 days of start; chest X-ray if positive test within 1 year
• Annual TB questionnaire required
• Immunity or vaccination documentation for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella (Chickenpox)
• Drug screening within 30 days of start (10-panel, no instant tests accepted)
• Seasonal flu vaccination required by November 1 (declinations accepted with mask requirement)

Facility Specific Requirements

• Photo for badge (headshot, shoulders up, plain white background)
• Completion of Code of Conduct Acknowledgement and other onboarding documents prior to start
• Fit test onsite after orientation
• Completion of new employee information sheet within 48 hours of contract signing

Additional Notes

• Candidates must notify management and orientation staff if they have latex allergies
• Flu shot is available onsite weekly from September to October
• Drug screen and health documents must be kept current throughout the contract
