Medical Social Worker - Travel/Contract
Location: Worcester, Massachusetts
Contract Length: 13 weeks
Job Summary
Provide behavioral health social services including comprehensive client assessments, diagnostic formulation, treatment planning, crisis intervention, and care coordination. Must be flexible with hours and locations within a 35-mile radius, with travel required between sites.
Work Schedule
- Day shift options:
- 5 days/week, 8 hours/day (10:00 AM – 8:30 PM)
- 4 days/week, 10 hours/day (10:00 AM – 10:30 PM)
- 40 hours guaranteed per week
- Weekend and float flexibility required
Required Qualifications
- Valid Licensed Clinical Social Worker (LCSW) license in Massachusetts
- Minimum of 1 year of recent experience as a Licensed Clinical Social Worker
- Master's degree required
- Ability and willingness to travel to multiple locations within a 35-mile radius daily
- Valid driver’s license and ability to transport as needed
Major Responsibilities
- Conduct initial comprehensive behavioral health assessments including clinical interviews and collateral contacts
- Develop diagnostic formulations and treatment recommendations
- Engage clients in safety and crisis planning; provide crisis intervention and de-escalation
- Assess eligibility for services and medical necessity
- Create treatment plans incorporating client strengths, preferences, and goals
- Make referrals and coordinate care with other service providers
- Document all service delivery and complete required consents and releases in compliance with program standards
- Complete additional outcome tools and assessments as required
Compliance and Documentation Requirements
- 7-year criminal background check within 30 days of start, including SSN trace, county searches, national criminal search, and education verification
- Massachusetts Statewide iCORI clearance within 30 days prior to start
- Physical exam within 12 months of start, including physician statement of fitness without restrictions
- Drug testing within 30 days of start (specific drug panels required)
- Immunizations or valid declinations for:
- COVID-19 (most recent vaccine or declination)
- Flu seasonal vaccine or declination (required during flu season)
- Tetanus/Diphtheria/Tdap
- MMR (Measles, Mumps, Rubella)
- Varicella
- Hepatitis B
- Tuberculosis testing: Two PPDs or equivalent within required timeframes; chest x-ray and TB questionnaire if positive
- 2 professional references within 3 years
- Employment application documenting 7 years of employment history with explanations for gaps over 30 days
- Skills checklist and competency exams as applicable
Additional Notes
- Preference given to candidates with no Return to Office (RTO) days; up to 5 days RTO allowed
- Advanced notice will be provided for daily assignment locations
- Flexibility and willingness to float among all community health locations required
