Job Opportunity: Registered Nurse (Hospice RN Case Manager)
Location: Newport News, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Position: Hospice RN Case Manager
- Schedule: Monday to Friday, plus every 3rd weekend rotation
- Shift Times: 8:00 AM to 4:30 PM or 9:30 AM to 5:30 PM
- Caseload: 15-18 patients, with a point system for visits and admissions (admissions count as 3 points, visits as 1 point)
- Patient Setting: Patients are located in private homes and facilities (LTC, AL)
- Responsibilities:
- Manage symptoms and provide pain management for hospice patients
- Conduct Hope Assessments
- Participate in weekly interdisciplinary team (IDT) meetings
- Experience Required: Minimum 1 year of RN Hospice Case Management experience in the field
Unit Description
- Focus on supporting patients in their end-of-life journey
- Care provided in home settings including private residences, long-term care, and assisted living facilities
Required Certifications & Competencies
- Current Basic Life Support (BLS) certification (online CPR not accepted)
- Advanced Cardiac Life Support (ACLS) may be required depending on unit
- Other certifications (PALS, NRP, EFM) to be confirmed based on unit needs
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exams relevant to specialty
- Pass/Fail Testing (EKG/Medication) during orientation
- Annual CEUs related to stroke education may be required depending on unit
Licensure & Background Requirements
- Valid RN license for Virginia
- Government-issued photo ID and current driver’s license required (mandatory for all Home Health & Hospice positions)
- Background Check:
- 7-year criminal history check covering all locations lived/worked in past 7 years
- Sex offender registry check
- DMV driving record with minimum 3 years driving experience (minimum age 21, 25 if transporting patients)
- Fingerprinting: Virginia VSP National Criminal Background Check results required (effective 7/15/2025)
- Employment Application and 2 professional references required
Health & Safety Requirements
- Physical exam within the last 12 months
- Negative TB test or chest X-ray within the last 12 months (or physician-signed clearance if positive)
- Required vaccinations or proof of immunity:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (series, titer, or declination)
- Varicella (Chickenpox)
- Seasonal Flu vaccination required during flu season (10/1 - 3/31) with strict deadlines for documentation and exemptions
- COVID-19 vaccination preferred (initial series)
- Drug screening within the last 12 months
Facility Specific Requirements
- Photo for badge (recent, professional, clear, and meeting specific guidelines)
- Confidentiality Agreement
- Scrub color policy acknowledgment
- Safety brochure review and acknowledgment
- Dress code policy acknowledgment
- Drug-free workplace policy acknowledgment
- Hand washing hygiene policy review
- Sworn disclosure for Lifelong Care, Home Health, Hospice, and Pharmacy positions (updated annually)
- Virginia State Police Check (notarized affidavit required prior to start)
- TB questionnaire updated annually
Additional Notes
- Candidates must be prepared to complete orientation including computer-based learning (CBL) with multiple attempts allowed
- All documentation must be completed and uploaded prior to start date as per facility and system requirements
