Job Opportunity: LVN / LPN - Travel/Contract

Location: Utica, New York
Contract Length: 13 weeks


Position Summary

Licensed Practical Nurse (LPN) needed for a rehabilitation and nursing center with experience in rehab nursing and long-term care (LTC).


Requirements

  • Experience: Minimum 1.5 years in LPN specialty (Rehab nursing center and LTC experience required)
  • Licensure: Valid New York LPN license
  • Schedule: Every other weekend
  • EMR: PointClickCare preferred
  • Travel Experience: Not specified (no prior travel experience required)
  • Local Radius: 75 miles (local candidates accepted)

Certifications & Competencies

  • ACLS (Advanced Cardiac Life Support) – AHA strongly preferred
  • BLS (Basic Life Support) – AHA strongly preferred
  • PALS (Pediatric Advanced Life Support) – AHA strongly preferred
  • TNCC (Trauma Nursing Core Course) – Required for ER staff (if applicable)
  • Medication Test/Pharmacology Competency Exam (80% passing score or meet/exceed Angoff score)
  • Competency Exam (minimum 80% passing score or meet/exceed Angoff score)
  • Dysrhythmia/EKG Competency Exam (for ICU, PCU, ER, CT ICU, and certain units; expires every 3 years)

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form (prior to start)
  • Background Checks (standard 7-year search, EPLS/GSA/SAM, HHS/OIG, Education Verification)
  • State License Verification (primary source online verification)

    • Photo for badge (headshot, plain background)
    • Online learning modules completed prior to first shift
    • Paper orientation documents
    • Code Elopement Handout
    • Code of Conduct
    • New Employee Orientation Booklet

    • Omnicell Form (for nursing staff)

Health & Safety Requirements

  • Physical exam within 1 year of start
  • Fit Mask test within 1 year and updated annually (specific N95 masks required)
  • Negative TB test or Chest X-Ray within 1 year (PPD, Quantiferon Gold acceptable; Tspot not accepted)
  • Immunizations or positive titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (titer, vaccine, or documented chickenpox history)
  • 9 Panel Drug Screen within 1 year of start
  • Seasonal Flu Vaccination or Declination (required during flu season)
  • Pneumococcal Vaccination or Declination (required for all rehab travelers)

References & History

  • 2 Professional References required for Travel, Allied, Per Diem Acute Care
  • 1 Professional Reference required for Per Diem Non-Acute Care
  • Not required for Non-Clinical roles
  • References must be within 3 years of start date
  • Annual Skills Checklist required
