Overview

• Travel/Contract role in Seymour, Indiana
• 13-week assignment
• Night shift: 3x12 hours, 18:00–6:00
• Work Labor Day and weekend rotation
• Float within scope as needed
• Local candidates accepted within 80 miles
• First-timers accepted depends on experience
• Pending state license not accepted
• RTO: call out if more than a week

Required Qualifications

• Valid RN license in Indiana
• ICU - Intensive Care Unit (2 years of experience)
• ICU - Intensive Care Unit experience within the last year
• ACLS issued by American Heart Association or American Red Cross
• BLS issued by American Heart Association or American Red Cross
• NIHSS
• PALS issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License

Preferred Qualifications

• CCRN
• CMSRN
• RN-BC

Additional Requirements

• Background check within 1 year of start, covering all locations lived/worked/in school, with annual update
• EPLS/GSA/SAM
• HHS/OIG
• Education verification, diploma, or transcript required
• Orientation Safety Manual acknowledgement annually
• State License 1 verification
• Medication Test/Pharmacology Competency Exam
• Competency Exam within 12 months of start, updated annually
• Review and sign confidentiality statement
• Review and sign attestation of eligibility
• Review and sign self-learning orientation packet acknowledgement
• Complete assigned computer-based learning modules
• Hep B vaccine declination if declining Hep B vaccine or unable to provide documentation
• Physical exam within 1 year of start
• Tdap
• Negative TB or chest X-ray within 1 year prior to start
• Rubeola, Mumps, Rubella: 2 MMR vaccines or titer
• Hepatitis B: positive titer, 3-dose vaccine series with positive titer, or declination
• Varicella: 2 Varivax vaccines or titer
• 9-panel drug screen within 1 year prior to start
• 2 professional references from within the past 3 years
• Skills checklist annually
