Job Details

• Start Date: ASAP
• Shift: 12-hour shifts; start times vary (7am, 9am, 11am)
• Contract Length: 13 weeks
• Night Shift: May need to cover one night shift during contract
• Weekend Requirement: Yes, 3 weekends per schedule
• On-Call: One weeknight per week and every 2nd or 3rd weekend (Sat or Sun) from 7am to 7am with 20-minute response time
• Block Schedule: Not accommodated

• Guaranteed Hours: Facility may call off 1 shift per 2 weeks

Experience & Case Types

Certifications & Licenses

• BLS: Required (AHA or Canadian Heart & Stroke Foundation certification accepted)
• National Certifications: Not required
• State License: Must be current and valid (no pending licenses accepted)

Required Documentation & Compliance

• Background Check: Must be completed within 45 days of start, covering all locations lived/worked in past 7 years; updated annually
• Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations & Health Requirements:

  • TDAP (within 10 years or declination accepted)
  • Negative TB test or chest X-ray within 12 months (annual updates required)
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Seasonal Flu vaccine or declination (required during flu season)
  • COVID-19 vaccine or declination (required if onsite)
• Fit Mask Test: Required for travel starts after 2/5/2024; must be updated annually
• Respirator Safety Clearance: Required for travel starts after 2/5/2024; annual clearance needed

Scheduling & Work Conditions

• Shift Length: 12 hours
• Start Times: 7am, 9am, or 11am
• On-Call: One weeknight per week and every 2nd or 3rd weekend
• Weekend Coverage: 3 weekends per schedule
• Floating: May be required to float to other Providence facilities within reasonable distance

• Competency Exams: Required as applicable by profession/specialty

References & Employment History

• References: Minimum 2 supervisor references covering 1 year of employment within last 3 years in comparable specialty/setting
• Employment Application: Must include at least last 5 years of employment with month/year format; 7 years for APN/Subcontractors

Additional Notes

• All requested time off (RTO) must be submitted at time of application and approved based on facility needs
• Facility may call off one shift every two weeks
• Locals and candidates with pending state licenses are not accepted for this assignment

This position is ideal for Surgical Techs with at least 1 year of experience in outpatient and community hospital settings, especially those with orthopedic, neuro, and vascular case experience, who are open to travel assignments with weekend and on-call requirements.
