Physical Therapist - Outpatient (Travel/Contract)
Location: Surprise, Arizona
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 4:30 PM
Hours per Week: 40 hours
Job Summary
Provide physical therapy evaluation and treatment for outpatient physical therapy patients, including injured workers. Supervise physical therapy assistants and technicians, perform various functional and ergonomic evaluations, and maintain communication with referring physicians and rehab nurses. Participate in marketing and community outreach as needed.
Key Responsibilities
- Evaluate and treat patients per physician referral or direct access
- Supervise physical therapy assistants and technicians according to PT plan of care
- Perform post-offer testing (e.g., WorkSTEPS, APET), Work Hardening Evaluations, Functional Capacity Evaluations, ergonomic evaluations, and job analysis
- Utilize modalities and exercise equipment available in the facility
- Manage scheduling, referrals, and phone communications as needed
- Maintain accurate billing codes for various payer types
- Provide weekly patient progress updates to rehab nurses and timely notes to referring physicians
- Maintain professional relationships with patients, physicians, case managers, and other contacts
- Assist with business development and marketing efforts when patient load allows
- Participate in community events representing the facility
- Perform additional tasks as required for operational success
Required Qualifications
- Valid Physical Therapist license in Arizona
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in an outpatient setting
Compliance and Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Emergency contact form completed prior to start
- Background checks including 7-year location/work/school history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG (updated annually)
- Copy of diploma or transcript
- Physical exam within 1 year of start date
- Negative TB test or chest X-ray within 1 year (with specific protocols for past positive TB)
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
- 9-panel drug screen within 1 year of start
- Seasonal flu vaccination or declination during flu season (October 1 – March 31)
- Annual skills checklist and competency exams as applicable
- Employment application/resume and 2 professional references (for travel, allied, or per diem acute care roles)
