Job Opportunity: Rehab Nurse (Registered Nurse) - Travel/Contract

Shift Details
- Night shift
- 3 shifts per week, 12 hours each (18:30 to 7:00)
- Total 36 hours per week

- General Rehabilitation
- Stroke Rehabilitation
- Spinal Cord Rehabilitation
- Brain Injury Rehabilitation
Staffing Ratio
- 1 nurse to 5 patients
Requirements
- Minimum 2 years of Registered Nurse experience
- Prior travel experience required (No first-time travelers accepted)
- Epic electronic health record experience required
- Weekend shifts required
- Float requirement applies
- Call not required
Required Certifications
- Basic Life Support (BLS)
- Must obtain or maintain all required certifications relevant to the unit (verify unit-specific requirements)
Licensure
- Valid Registered Nurse license for Louisiana required
- License verification must be completed within 30 days of start
Competencies
- Medication/Pharmacology competency exam (within 12 months, updated annually, minimum 80% score)
- Dysrhythmia/EKG competency exam (within 12 months, updated annually, minimum 80% score)
- Core competencies (Parts I, II, III) required and must be updated annually
Background and Compliance
- Background check within 6 months covering all locations lived/worked/schooled in past 7 years; must include social security trace, county, nationwide, sex offender, federal extended, and FACIS Level 3 checks; updated annually
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Sex offender search processed by client request
- Government-issued photo ID required with name matching all documentation
- Employment application must include 7 years of work history with no gaps over 30 days
Health and Immunization Requirements
- Physical exam within 12 months (not required by employer but required by placement agency)
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declinations accepted with proper documentation)
- Negative TB test or chest X-ray per current guidelines (Qgold/Tspot or 2-step skin test within 3 months of start)
- Annual TB questionnaire required
- Measles, Mumps, Rubella (MMR): proof of 2 vaccines or positive titer; if low titer, 2 boosters required (must complete within 60 days if started without boosters)
- Hepatitis B series, titer, or declination (declinations must be signed within 12 months of start)
- Varicella: proof of 2 vaccines or positive titer; if low titer, 2 boosters required (must complete within 60 days if started without boosters)
- Drug screening within 30 days of start (specific panels depending on hire date)
- Seasonal flu vaccination/documentation required annually (Oct 1 – Mar 31), with exemptions and declinations accepted under specified conditions; mask required if unvaccinated
