Job Opportunity: Case Manager (Registered Nurse)

Job Description

• Coordinate case management from patient admission through post-discharge, including outpatient settings.
• Monitor patient care progress and address patient and family needs.
• Resolve obstacles to effective care and coordinate with payers, vendors, patients, and families.
• Collaborate with internal team members to establish, monitor, and execute patient discharge plans.
• May be required to access and administer medications within scope of practice and according to Utah state law.
• Work schedule: Monday-Friday, 8:00 AM to 4:30 PM (5 days a week, 8-hour shifts).

Required Qualifications

• Valid Registered Nurse (RN) license in Utah.
• Minimum 1 year of case management experience within the last year.
• Experience in an acute care setting, specifically acute hospital case management.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Compliance and Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID).
• E-Verify compliance required.
• Background check within 6 months prior to start, including social security trace, county, nationwide, and sex offender checks.
• Monthly updates for EPLS/GSA/SAM and HHS/OIG checks after hire.
• Annual skills checklist.
• Immunization and health documentation including:

  • Flu vaccine (seasonal, with medical/religious exemptions considered case-by-case).
  • Tdap vaccine within 10 years.
  • Negative TB test or chest X-ray per facility protocol.
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer.
  • Varicella vaccination or positive titer.
  • Hepatitis B surface antibody titer showing immunity (vaccine series alone not accepted).
• Drug screening: 11-panel drug test including MDMA within 30 days of start; valid for two years.
• Physical exam within 1 year of start.
• Respirator medical clearance and fit testing if applicable.

Additional Notes

• Locals are not accepted for this travel contract.
• Candidates with latex allergies will have access to non-latex gloves.
• Exemptions for immunizations require facility-specific forms and approval prior to start.
• References: 3 professional references required, including at least one supervisor or manager reference.
